Job summary

Supportability Support Coordination Service is seeking a dedicated Support Coordinator in the Greater Brisbane area for a part-time/contract role. You will work with clients to implement supports and coordinate services to improve independence and quality of life.

Qualifications

• Minimum 2+ years in a support coordination role.
• Preferred tertiary qualifications in human services or allied health.
• Knowledge of NDIS Act 2013 and related guidelines.
• Ability to work with clients, families and carers in the community.

Responsibilities

• Support clients to achieve goals in their NDIS plans.
• Coordinate funded and mainstream supports.
• Strengthen informal networks and client independence.
• Collaborate with the support coordination team to deliver outcomes.
• Prepare clear reports and maintain case records.
