Job summary

Your Support Coordination is seeking an experienced Support Coordinator in Swan Hill, Victoria. This permanent part-time role—four days per week—offers the chance to manage your own caseload and work with a small regional team that backs you with real supervision and support.

You will use your NDIS knowledge to hit the ground running, support participants, and coordinate with families and providers. A local, meaningful role awaits in a supportive community team.

Qualifications

• Experience as a support coordinator in the NDIS.
• Current NDIS Worker Screening Check.
• Working with Children Check or willingness to obtain both before starting.
• Current driver licence and access to reliable vehicle.

Responsibilities

• Hold your own caseload of NDIS participants across Swan Hill and surrounding districts.
• Help participants understand their plans and connect them with providers and supports.
• Build participants' capacity to make informed funding choices.
• Monitor plan budgets and act early on underspend/overspend.
• Maintain accurate case notes, service agreements and records per NDIS standards.
• Keep billable hours at or above 80% of scheduled hours.
