Job summary

BaptistCare is seeking a compassionate Case Manager to support people aged 18 to 64 across the Adelaide metropolitan area. The role focuses on strengths-based, person-centred care to help participants reduce loneliness, build social connections and access community opportunities.

Multiple 0.5 FTE positions are available across North, South and East regions, on a maximum term basis until 30 June 2028. Standard hours 8:30am–4:30pm with some day flexibility; Wednesday is mandatory.

Qualifications

• Passionate and empathetic professional
• Two years' case management experience
• Experience applying social work theories to case management
• Ability to collaborate with participants, families and stakeholders
• Understanding of strengths-based and trauma-informed practice

Responsibilities

• Provide strengths-based, person-centred support to engage participants in meaningful activities
• Build relationships with individuals, families and community partners
• Assist participants to access community opportunities and improve wellbeing
