Job summary

BaptistCare is seeking a compassionate Case Manager to support people aged 18 to 64 across the Adelaide metropolitan area. The role focuses on strengths-based, person-centred care to help participants reduce loneliness, build social connections and access community opportunities.

Multiple 0.5 FTE positions are available across North, South and East regions, on a maximum term basis until 30 June 2028. Standard hours 8:30am–4:30pm with some day flexibility; Wednesday is mandatory.

About the Role – Multiple Part-Time Maximum Term Opportunities (until 30 June 2028)

Join our Community Connections team and play a meaningful role in supporting people aged 18 to 64 to build social connections, reduce loneliness and increase their sense of belonging within the community. As a Case Manager, you will provide strengths-based, person-centred support that helps participants engage in meaningful activities, build relationships and access community opportunities that enhance their wellbeing and quality of life. Working closely with individuals, families and community partners, you'll contribute to creating positive and lasting outcomes for people across the Adelaide metropolitan community.

Multiple part-time opportunities are available from 0.5 FTE across the North, South and East metropolitan regions. These positions are offered on a maximum term basis, expiring on 30 June 2028. Standard hours are 8:30am to 4:30pm, with flexibility around working days to suit both operational requirements and the successful candidate. Please note that Wednesday is a mandatory working day for all positions.

About you:

You are a passionate and empathetic professional who enjoys working alongside people to achieve meaningful outcomes. You build strong relationships, take a person-centred approach and are confident supporting individuals with diverse needs. You are organised, proactive and committed to helping people strengthen social connections, engage with their community and improve their overall wellbeing.

You will also have the following skillsand experience:

• Tertiary qualifications relevant to the work such as a Bachelor of Social Work, Social Sciences, Community Development or equivalent.
• Minimum two years' experience in case management.
• Experience applying social work theories and models to case management, including restorative and reflective practice.
• Ability to work collaboratively with participants, families and community stakeholders.
• Sound understanding of strengths-based and trauma-informed practice.
