Job summary

BaptistCare is seeking a Care Consultant to work in Geelong, providing person-centred in-home care. You will assess needs, plan services, manage budgets and visit clients to ensure effective care in their own homes.

Qualified in Case Management or Nursing/Allied Health/Social Work, you’ll thrive with autonomy, travel across the region, and contribute to a values-led team with strong client relationships.

Qualifications

• Must have a qualification in Case Management or an undergrad in Nursing, Allied Health or Social Work.
• Experience in case management or home/community services is highly regarded.
• Strong organisational skills to manage a complex caseload independently.

Responsibilities

• Handle intake, initial and ongoing assessments of home care needs.
• Develop and maintain goal-directed care plans within the customer’s home care budget.
• Coordinate supports and services across the funded program.
• Manage package spend and advocate for customers’ needs and preferences.
• Conduct regular home visits and monitor care effectiveness.
• Build relationships with customers, families and community stakeholders.

Full Time | 12-Month Max Term Contract | Hybrid | Salary Packaging Available

As a Care Consultant at Baptistcare, you won’t be processing people through a system. Instead, you’ll be walking alongside them, helping them stay safely and independently in the homes they love.

This is a 12-month Max term contract covering Geelong and surrounding areas within our In-Home Care (Support at Home) program. If you’re an experienced case manager or allied health professional who thrives on autonomy and genuinely person-centred work, keep reading.

THE ROLE

You’ll be the primary point of contact for a caseload of home care customers assessing their needs, planning their services, managing their budgets, and visiting them regularly to make sure their care is actually working for them.

• Conduct intake, initial, and ongoing assessments of aged care-related needs
• Develop and maintain goal-directed care plans within each customer’s home care budget
• Coordinate and facilitate supports and services across the funded program
• Manage package spend and advocate for customers’ needs and preferencesConduct regular home visits and ongoing monitoring to support customers to age well in place
• Build strong relationships with customers, families, and community stakeholders

ABOUT YOU

You bring a mix of clinical or case management foundations, genuine empathy, and the organisational skills to manage a complex caseload independently.

• Qualification in Case Management, or an undergraduate qualification in Nursing, Allied Health, or Social Work (essential)
• Prior case management experience, particularly in home care or community services (highly regarded)
• Experience completing initial and annual customer assessments and care planning
• Knowledge of the Support at Home program and the broader home care sector
• Division 1 Nursing registration (desirable but not essential)
• Based in Mornington peninsula with a current driver’s licence and willingness to travel across the region
• National Police Check (or willingness to obtain)
• Proof of working rights in Australia
