Job summary

2b Inclusive is seeking an experienced Social Worker to join our Pathways to Success Program, supporting people living with disability who experience complex mental health and psychosocial needs.

This hands-on case management role suits someone who enjoys building relationships and helping people navigate often difficult-to-access systems. You will travel across metropolitan Adelaide and regional South Australia to support participants.

Qualifications

• Bachelor or Master of Social Work is required.
• Experience as a Social Worker.
• Experience supporting people with complex mental health, psychosocial or social needs.
• Strong case management, advocacy and service-navigation skills.
• Confidence working with complex situations and multiple service systems.
• Excellent communication and relationship-building skills.
• Ability to work independently while knowing when to seek support or supervision.
• Practical, persistent and person-centred approach.
• Experience in community mental health, psychosocial disability, housing and homelessness, justice, child and family services, AOD, NDIS or regional services is highly regarded.

Responsibilities

• Provide intensive, hands on, person-centred case management.
• Support people experiencing complex mental health, psychosocial and disability-related needs.
• Complete assessments and develop practical case management goals.
• Help participants access mental health, health, housing, disability and community services.
• Advocate with and for participants where barriers to services exist.
• Coordinate with GPs, mental health teams, government agencies, community services and other professionals.
• Provide assertive engagement and outreach for people who may find it difficult to engage with services.
• Attend case conferences and multidisciplinary meetings.
• Maintain clear, professional and timely case notes and documentation.
• Travel within metropolitan Adelaide and regional South Australia; regular regional travel including Riverland as needed.
