Personalised Care in Liverpool is seeking a compassionate Support Worker to empower disabled participants within the NDIS community. The role involves providing one-on-one support, facilitating community access, and implementing personalised plans to enhance independence and overall wellbeing.

Qualifications

• Previous experience as a Support Worker or in a similar role is preferred, but not essential.
• Excellent interpersonal and communication skills, with ability to build rapport with participants and families.
• Compassion, empathy and patience in providing person-centred care.
• Flexibility to adapt to changing circumstances and participant requirements.
• Ability to work collaboratively as part of a team.
• A valid driver’s license and access to a vehicle is preferred.

Responsibilities

• Facilitate community access and engagement for disabled participants, enabling them to lead independent and fulfilling lives.
• Provide one‑on‑one support to participants, promoting their safety, well‑being, and overall quality of life.
• Assist participants in developing life skills, fostering social connections, and accessing community resources.
• Implement individualised support plans, tailoring assistance to meet the unique needs and goals of each participant.
• Encourage and empower participants to pursue their interests, hobbies, and personal development opportunities.
• Foster a nurturing and inclusive environment, promoting independence, self‑advocacy, and community integration.
• Document participant progress and maintain accurate records in accordance with organisational guidelines.
