Job summary

Fair Foundations is seeking a proactive Support Coordinator to work with NDIS participants in Fairfield and Greater Western Sydney. You will provide person-centred, trauma-informed capacity-building support, help participants implement their plans, and connect with mainstream and provider supports.

Ideally you hold a Certificate IV or Diploma in Disability or related fields, with 3+ years experience coordinating supports for people with complex needs, and you meet NDIS standards and screening

Qualifications

• Experience coordinating supports for people with complex needs.
• Knowledge of NDIS Quality Standards and principles.

Responsibilities

• Support participants to prepare for plan reviews.
• Coordinate service agreements and funding allocations.
• Maintain accurate case notes and reporting.
• Build relationships with participants, families, providers and community stakeholders.
