Job summary

Fair Foundations is seeking a Support Coordinator to provide person-centred, trauma-informed capacity-building support to NDIS participants, including those with complex needs. You will help participants understand and implement their plans, address barriers, connect with mainstream, community and provider supports, and build confidence to coordinate and manage services.

Reporting to the Head of Disability Services, the role includes preparing participants for plan reviews, coordinating funding

Qualifications

Certificate IV, Diploma or Advanced Diploma in Disability, Community Services, Mental Health, Welfare, Mental Health Peer Work or Counselling.
At least three years’ experience providing support coordination to people with complex needs.
Sound knowledge of NDIS Quality Standards and principles; experience implementing participant plans.

Responsibilities

Coordinate and support participants to prepare for plan reviews and assessments.
Arrange service agreements and funding allocations; maintain case notes and reporting.
Build relationships with participants, families, providers and community stakeholders.
Develop and sustain a Support Coordination caseload that meets targets.
