Job summary

Superior Care Solutions in Sydney seeks an experienced Support Coordinator to link NDIS participants with needed supports and advocate for their rights. You will guide participants through plans, budgets, and system navigation, with a focus on person-centred care in a hybrid office/remote role.

The role involves managing a caseload of 15-20 participants, ensuring timely plan reviews, and collaborating with families, schools, and health providers. Travel within ~30 km is required.

Qualifications

• NDIS Worker Screening Check
• First Aid certification
• CPR certification
• NDIS Worker Induction Modules completed
• Working With Children Check (WWCC)
• Driver’s licence (current) and access to a personal vehicle
• Full comprehensive car insurance
• Right to work in Australia
• Minimum 2 years’ experience in support coordination or closely related role
• Comprehensive knowledge of the NDIS framework and funding categories
• Excellent written and verbal communication
• Strong organisational and time management skills

Responsibilities

• Implement participant NDIS plans, goals and funding categories.
• Help participants and families understand NDIS plans, budgets, and the system.
• Identify and connect participants with services aligned to plan goals.
• Build participants’ capacity to manage supports independently over time.
• Prepare comprehensive progress reports for plan reviews.
• Support participants through plan reviews and reassessments.
• Advocate when participants need a stronger voice in the system.
• Liaise with service providers, families, schools, allied health and other stakeholders.
• Maintain detailed case notes and meet NDIS reporting requirements.
• Identify gaps in local services and help participants find alternatives.
• Provide crisis support and handle escalations when things go wrong.
