Job summary

Approved by Frankie in Sydney is seeking an experienced NDIS Support Coordinator to help participants understand and implement their plans, build capacity, and coordinate services across disability, health, housing, justice and community systems.

You will identify risks and safeguarding concerns, respond to changes in circumstances, maintain clear case notes, and work collaboratively while keeping the participant’s goals, rights and preferences at the centre.

Qualifications

• Experience in NDIS Support Coordination or Specialist Support Coordination.
• Ability to navigate the NDIS system confidently.
• Capacity to manage competing priorities while keeping the person at the centre.
• Strong communication and relationship-building across diverse backgrounds.
• Commitment to ethical, independent practice and identifying safeguarding concerns.

Responsibilities

• Help participants understand and implement their NDIS plans.
• Build participant capacity, choice and control.
• Coordinate services across disability, health, housing, justice and community systems.
• Identify and respond to risks, service gaps and safeguarding concerns.
• Support participants through reviews, changes in circumstances and crisis or transition.
• Maintain clear, timely and professional case notes and documentation.
• Build effective relationships with participants, families, guardians, providers and government agencies.
• Work collaboratively while focusing on the participant's goals, rights and preferences.
