Qualifications

• Experience in NDIS Support Coordination or strong case management in disability, aged care, community services or related sector.
• Clear written communication and good record keeping.
• Current NDIS Worker Screening Check and Working with Children Check, driver’s license and comprehensively insured vehicle.

Responsibilities

• Meet participants and families, work through their plan and set up the supports it funds.
• Source and connect providers, chase waitlists and ensure services are in place.
• Track plan budgets so funding lasts the length of the plan.
• Be the point of contact when issues arise with providers or services.
• Support participants through plan reassessments, including evidence gathering and reports.
