Job summary

Align Care seeks a dedicated coordinator to assist participants with understanding and implementing their NDIS plans. You will coordinate services and connect participants with appropriate providers to support independence and goal achievement.

You will liaise with participants, families, carers, the NDIA and other stakeholders to ensure quality outcomes and timely reviews, maintaining accurate case notes and reports throughout.

Responsibilities

• Assist participants to understand and implement their NDIS plans.
• Coordinate and connect participants with appropriate service providers.
• Support participants to achieve their goals and build independence.
• Monitor supports, budgets, and service delivery to ensure quality outcomes.
• Liaise with participants, families, carers, the NDIA, and other stakeholders.
• Prepare participants for NDIS plan reviews and obtain supporting reports and assessments.
• Advocate for participants and coordinate multidisciplinary supports.
• Maintain accurate case notes, reports, and participant records.
• Identify and manage risks, safeguarding concerns, and changes in participant needs.
• Ensure services are delivered in accordance with the NDIS Quality and Safeguards Commission Practice Standards, organisational policies, and a person-centred approach.
