Job summary

OnSide is seeking a dedicated Support Coordinator to help clients navigate the NDIS plan and access community services. You will work with children, young people and adults to gather resources and support goals.

The role is permanent, with flexible hours (full time or 0.8 FTE 4 days when eligible), hybrid work from home and client visits across the Gold Coast. NDIS training and vehicle insurance included.

Qualifications

• Ability to build relationships while coordinating supports within the NDIS.
• Strong organisational and time-management skills for multi-client coordination.
• Commitment to high-quality client service and accessibility.

Responsibilities

• Build clients' capacity and support their NDIS plan goals.
• Coordinate supports across multiple providers and services.
• Support client independence across daily life activities.
• Manage and monitor funding budgets for each client, reporting as needed.
• Gather and document information for evidence-based reports and updates.
• Collaborate with stakeholders to link services and achieve client goals.
