ConneXtions is a not-for-profit organisation in Victoria dedicated to helping people with disabilities live inclusive lives. We seek a part-time Support Coordinator in Wodonga to assist participants with NDIS planning, access to services, and coordination of supports.

You will collaborate with participants, families and providers to implement plans, monitor funding, and prepare for reviews, while upholding person-centred care and community engagement.

Formación

• Relevant tertiary qualification in social work, mental health, or community services.
• Minimum 3 years' experience in a similar support coordination or case management role.
• Experience with complex care.
• Strong understanding of the disability support sector, including legislation, policies, and funding schemes.
• Current Australian Driver’s Licence with access to a car and full comprehensive insurance.
• NDIS Worker Screening Check

Responsabilidades

• Support participants and their networks to understand and implement their NDIS plans and the reasonable and necessary principles.
• Work with participants to connect with community services and engage quality providers.
• Strengthen and maintain informal support networks for participants.
• Monitor funding and utilisation to maximise value for money.
• Assist participants to manage and direct their own supports where appropriate.
• Prepare for NDIS planning meetings and gather evidence to support access to services.
• Arrange any assessments required to determine funding needs.
• Be the key contact for service issues, complaints, plan changes and reviews; support goal progress reviews.
• Develop service agreements and create bookings with providers.
