Job summary

Total Support Services is seeking an NDIS Support Coordinator in Adelaide to empower clients with disabilities to access services and manage their plans. The role focuses on assisting participants with mental health needs to navigate the NDIS system and coordinate supports with providers.

You will work from our Adelaide office, engaging with participants, families and service providers to deliver person-centred outcomes in a collaborative, supportive team environment.

Qualifications

• Tertiary qualifications in Disability or equivalent training, with at least two years' related experience.
• Demonstrated experience working with clients with mental health needs is essential.
• Previous experience in NDIS Support Coordination and a sound understanding of NDIS plans and participant funding.

Responsibilities

• Empower clients by helping them connect with communities and service providers to achieve their goals.
• Assist clients to understand the NDIS scheme, navigate their plan, and maximise funded supports.
• Collaborate with participants, families, service providers and professionals to deliver coordinated supports.
