Job summary

VMCH is seeking a dedicated Support Coordinator in Wangaratta to empower participants to navigate the NDIS and access appropriate supports. You will work within our Disability Services team to build strong relationships with participants, families, carers and providers, delivering person-centered plans and outcomes.

Qualifications

• Experience in the Disability sector or a relevant qualification in disability or community services.
• Strong knowledge of NDIS, Disability Act and related legislation.
• Experience in measuring and reporting client and program outcomes.
• Ability to work autonomously and as part of a team, managing competing demands.
• Proven ability to establish and maintain positive stakeholder relationships.
• Excellent written and verbal communication.
• Current NDIS Screening Clearance and Working with Children’s Check.

Responsibilities

• You are experienced in the Disability sector with strong knowledge of the NDIS.
• You will collaborate with participants, informal supports, and stakeholders to deliver person-centered support.
• Provide coaching and guidance to help participants navigate the NDIS and community resources.
• Organise and facilitate meetings with participants, families, and service providers.
• Monitor budgets and ensure services align with participants’ plans and goals.
• Respond to service gaps or crises, coordinating immediate interventions as needed.
