Job summary

LifeFul is seeking a dedicated Support Coordinator in the Melton area of Melbourne. You will work with NDIS participants to turn plans into action, building trusted networks and connecting them with the right services to improve independence and quality of life.

You will collaborate with Local Area Coordinators, providers, and community partners, ensuring high standards of care, accurate documentation, and compliant practice, with a focus on achieving outcomes and growth for participants and

Qualifications

Certificate III in Individual Support / Disability Support is mandatory.
NDIS Worker Screening Check, Working with Children Check, First Aid & CPR certificates are required.
Valid Australian Driver’s Licence and vehicle insurance.
Right to work in Australia.

Responsibilities

Transform participant goals into clear, achievable plans.
Connect participants with the right providers, supports and community opportunities.
Monitor progress, track outcomes, and ensure supports are delivered effectively.
Build and maintain relationships with LACs, providers, and stakeholders to enhance outcomes and drive referrals.
Identify growth opportunities for participants and LifeFul to help people thrive.
Uphold NDIS standards, safeguarding practices, and ensure 85% billable hours weekly.
