Job summary

Supporting Independent Living Co-operative (SILC) in Sydney is seeking a dedicated Support Coordinator to help participants implement their NDIS plans and access necessary services.

You will work with participants and families to understand funding categories, identify goals, connect to providers, manage budgets, and advocate for outcomes within a cooperative, person-centered framework.

Qualifications

• Minimum 1 year experience in the disability sector.
• Diploma in a related human services field.
• Knowledge of NDIS funding categories.
• Understanding of person-centered planning approaches.
• Ability to build rapport with participants, families and service providers.

Responsibilities

• Help participants understand their NDIS plan and funding categories.
• Identify participants' needs, challenges and goals.
• Connect participants to services and supports.
• Build skills to coordinate supports and services effectively.
• Coordinate with service providers across various support categories.
