Job summary

Better Support Solutions Pty Ltd. is seeking an enthusiastic Support Coordinator to help participants understand and implement their NDIS plans in Sydney, NSW. You will collaborate with participants to develop plans, coordinate services, and build supportive networks.

The role emphasizes strong communication, organisational skills, and a client-centered approach to empower participants in navigating the NDIS landscape.

Qualifications

• Qualification in Allied Health or social worker or related discipline.
• Sound understanding and knowledge of NDIS frameworks and funding.
• Minimum 1 year experience in a similar role.
• High level of organisational, time management and planning skills.
• Strong problem solving skills.
• Excellent communication (verbal and written).
• Valid full driver’s license (own vehicle is not required).

Responsibilities

• Assist the participant to develop knowledge, experience and connections with the community and broader systems of support.
• Work with the participant to understand plan funding and design support solutions to meet outcomes.
• Identify support options and link to broader systems; create supports and action plans.
• Provide crisis support and build capacity and resilience in the participant’s network.
• Coach the participant through challenges and prepare for reviews with reported outcomes.
• Provide information about the NDIS and help navigate its systems for participants and carers.
• Research providers and stakeholders to meet identified needs and build relationships.
• Prepare required reports for internal and external purposes.
• Other duties as directed by your manager.
