Job summary

Focused on Care Pty is seeking an experienced NDIS Support Coordinator to join our Kempsey/Port Macquarie team. The role involves assisting clients with their NDIS Plans and coordinating services, while building relationships with local providers and disability groups.

The successful candidate will work from our Kempsey office 2 days a week, with travel across the region as needed. A passion for person-centred care and achieving KPIs is essential.

Qualifications

• Experience in a support coordination role (essential).
• Strong client-centred approach with ability to build relationships.
• Ability to manage multiple clients and meet KPIs.

Responsibilities

• Assist existing and new clients to implement NDIS Plans and coordinate services.
• Build and maintain relationships with service providers and disability groups.
• Promote Focused on Care's support coordination services to grow the client base.
