Job summary

Abled Care and Nursing Services in Tuggerah, NSW, is seeking a NDIS Support Coordinator for a full-time role. You will coordinate NDIS-funded supports, liaise with participants and families, and monitor plans to ensure they meet changing needs.

Aiming to empower clients to live independently with person-centred care in a supportive team. The role requires at least 2 years in NDIS coordination, solid NDIS knowledge, and strong communication.

Qualifications

• Minimum 2 years' experience in NDIS support coordination
• Strong understanding of NDIS guidelines
• Excellent communication and interpersonal skills
• Client-focused problem solving
• Proficient with NDIS and client management software
• Tertiary qualification in allied health, social work or community services
• Current driver's licence and reliable vehicle
• Able to use Astalty software
• Maintains 20 billable hours KPI

Responsibilities

• Coordinate implementation of NDIS participant plans, ensuring funded supports and services are in place
• Liaise with participants, families and other providers to facilitate access to supports
• Monitor and review participant plans, adjusting as needed to meet changes
• Advocate for participants within the NDIS system
• Maintain detailed records to ensure NDIS compliance
• Collaborate with the Abled Care Services team to provide holistic support
