Job summary

UnitingCare Wesley Bowden is seeking a Specialist Support Coordinator to provide person-centred support to people with high or complex needs under the NDIS framework. You will develop personalised plans, manage budgets, and build strong links with providers to enable safe, effective services.

You will guide participants on NDIS plans, respond to crises, and complete required NDIA reporting with integrity. A trauma‑informed, inclusive approach and strong organisational skills are essential.

Job description

As a Specialist Support Coordinator, you will provide person‑centred support to people with high or complex needs within the NDIS framework. Your role will support participants through a “do with, not for” approach, helping them build confidence and strengthen their ability to manage their own supports.

Key responsibilities include developing personalised support plans and working within funding budgets. You will build strong relationships with service providers and stakeholders to support safe and appropriate service delivery, while helping to address complex barriers to access. You will provide guidance on using NDIS plans and support participants during periods of crisis. You will also meet reporting requirements to the NDIA, supporting transparency and accountability in service provision.

Does this describe you?

You are a compassionate and organised professional with experience in complex case management or specialist support coordination. You understand how to support people with complex needs, including disability and mental health challenges. Your person‑centred approach and commitment to inclusion help you build trust and respond to each participant’s needs with care and respect.

You manage your time well, prioritise competing tasks and are comfortable working independently. You are practical and resourceful when solving problems. You also bring a strong commitment to cultural safety and inclusion, especially when working with diverse communities, including LGBTIQA+ people and Aboriginal and Torres Strait Islander peoples.

You hold a relevant tertiary qualification in an Allied Health discipline or similar. You can develop person‑centred support plans and help participants work towards their goals. Your knowledge of community networks and services helps you connect participants with suitable providers and supports.

You are confident using Microsoft Office and cloud‑based software to document and report on the services you provide. You maintain high standards in documentation and reporting. You also understand the regulatory environment and can liaise with the NDIA and NDIS Quality and Safeguards Commission as needed.

You work with integrity and transparency, building positive relationships with participants, families, providers and other stakeholders. You care about supporting people in ways that build confidence, choice and independence. Your trauma‑informed and strength‑based approach will help you make a meaningful difference in this role.

• Current DHS Working with Children Check (WWCC)
• Current Police Clearance
• NDIS Worker Screening Check
• Tertiary qualification in a relevant Allied Health discipline
