Job summary

Disability Plan Services is seeking experienced Support Coordinators to work in the Toowoomba/Brisbane region. You will support participants to understand and implement NDIS plans, coordinate services, build capacity and maintain accurate documentation.

This role offers a hybrid work model with a manageable caseload and flexibility to structure your day. A strong foundation in NDIS, plan implementation and compliance is essential for success.

Qualifications

• Minimum 12 months experience in Support Coordination.
• Proven ability to manage mixed caseloads independently.
• Strong understanding of the NDIS, plan implementation, compliance requirements and Budget Planning.
• Experience in a Human Services Setting.
• NDIS Worker Screening Check and Blue Card.
• NDIS Orientation Module, NDIS Effective Communication Module.

We are seeking experienced Support Coordinators to continue delivering consistent, person-centred support to participants in the Toowoomba / Brisbane region.

About the Role
You will support participants to understand and implement their NDIS plans, coordinate services, build capacity, resolve barriers and maintain accurate documentation. These roles require strong communication skills, sound judgement and the ability to work independently while managing a varied caseload.

What We’re Looking For

Minimum 12 months experience in Support Coordination - Essential

Proven ability to manage mixed caseloads independently - Essential

Strong understanding of the NDIS, plan implementation, compliance requirements and Budget Planning - Essential

Experience in a Human Services Setting

Ability to manage competing priorities and work autonomously

Relevant qualifications in Disability, Mental Health, Community Services, Social Work or Allied Health

Driver’s licence and reliable vehicle

NDIS Worker Screening Check and Blue Card

NDIS Orientation Module, NDIS Effective Communication Module
