Job summary

Our Support Coordination team helps participants make the most of their NDIS plans by connecting them with the right services, providers and resources. The successful applicant will assist participants in understanding and implementing their plans to achieve their goals.

You will work closely with individuals to identify needs, explore options, establish service agreements, and promote independence over time.

Qualifications

• Diploma or higher qualification in a relevant field.
• At least one year of work experience in accredited residential care or disability care.

Responsibilities

• Help participants understand their NDIS plan and funding allocations
• Coordinate with service providers and community resources
• Assist with problem-solving and overcoming barriers to accessing services
• Encourage independence and informed decision-making
• Liaise with government bodies and community groups about resources
