Job summary

Courage Support Services in Melbourne is seeking a dedicated support coordinator to help participants understand and optimise their NDIS plans. You’ll identify needs, connect participants with services, and support safeguarding and empowerment.

You’ll work with families and providers across education, health and community services, design practical plan actions, and help participants navigate funding to improve wellbeing, independence and resilience in a participant-centred team.

Qualifications

• NDIS knowledge, especially support coordination and choice and control.
• NDIS Worker Screening Check cleared.
• NDIS Worker Orientation Module completed.
• Disability-related qualification is desirable but not essential.

Responsibilities

• Connect participants with relevant services and supports.
• Help participants understand their plan funding and maximise it.
• Negotiate service agreements and set up appointments.
• Design support solutions and, where practical, an action plan.
• Reduce barriers to using the plan across education, health or justice services.
• Coach participants through challenges and support plan reviews.
