Job summary

VHCC seeks a Community Worker in Box Hill, VIC to support NDIS participants and their families. The role focuses on assessing needs, developing individualised plans, and connecting clients with funded supports within a multidisciplinary team.

The successful candidate will work in a newly created full-time position, coordinating services to help participants achieve independent living and goal attainment in the community.

Qualifications

• Australian citizenship or permanent resident status is preferred.
• Minimum 1 year of relevant experience in community services.
• Sound knowledge of NDIS processes, funding categories, and budget reporting.
• Ability to conduct needs assessments and prepare case notes and progress reports.
• Understanding of person-centred, trauma-informed and rights-based practice.
• Excellent communication and interpersonal skills; ability to work with diverse backgrounds.

Responsibilities

• Interview participants and families to assess needs and plan supports.
• Prepare, manage and implement individualised NDIS plans within budget and guidelines.
• Provide person-centred assistance and guide participants through the NDIS process.
• Maintain clear communication with participants, families and stakeholders; report progress to NDIS.
