Job summary

Community Accessability in regional Victoria is seeking an NDIS Support Coordinator to empower participants to navigate the NDIS and participate more fully in their community. You will manage a portfolio of participants, build rapport with clients and providers, and help set up service agreements, plan reviews, and risk assessments.

Qualifications

• Demonstrated experience in Support Coordination within the Community Sector.
• Experience in working with complex participants and families.
• Ability to initiate and maintain contacts with a broad range of people and community organisations to ensure our clients' access to services, appropriate communication with fellow workers, families, carers and other relevant stakeholders.
• Experience and knowledge of the NDIS, a good understanding of associated processes and support networks.
• The ability to deal with changes in the disability and aged care sectors.
• Exceptional verbal and written communication.
• An ability to work efficiently with a CRM database.
• Experience in a KPI-driven environment.
