Assist participants to work towards the goals outlined in their NDIS plans, monitoring progress and adjusting support strategies as needed.

Provide emotional support and maintain clear, effective communication with participants, families and stakeholders. Liaise with and report to the NDIS on participants’ progress and plans. Connect participants with appropriate services and funded supports, ensuring their needs are met effectively.

Ensure participant plans are implemented in accordance with the allocated budget and NDIS funding guidelines. Collaborate with internal teams and external services to maximise outcomes for participants through a coordinated and integrated support system. Assist participants to coordinate and implement supports that build their capacity for independent living.

Strong organisational, time management and problem-solving skills, with the ability to manage a varied caseload.
