Job summary

Claro Disability Services in Melbourne’s southeast is seeking a compassionate Support Worker to assist clients in accommodation settings. You’ll work with individuals who have complex needs, providing person-centered care, mobility support, daily tasks, and meaningful activities.

This role emphasizes collaboration with families and health teams, flexible shifts, and culturally respectful practice to help clients live more independent and connected lives.

Qualifications

• Experience in complex care, disability, or health settings.
• Excellent communication and teamwork.
• Ability to tailor supports to individual goals.

Responsibilities

• Chat with the client and family to understand goals and routines.
• Assist with mobility, transfers, daily tasks, and devices per plan.
• Monitor medical plans and liaise with the care team when plans shift.
• Provide social time and community access aligned with interests.
• Support decision-making and independence while respecting consent.
• Respect backgrounds, beliefs and cultures in daily care.
