Job summary

Coast & Country Primary Care Bathurst NSW is seeking a Social Worker (Mental Health) to join a multidisciplinary team delivering psychological therapies for underserviced populations across Bathurst and surrounding regions.

Responsibilities include psychosocial assessments, evidence-based interventions, care planning, risk management, care coordination and family engagement to support recovery and community participation.

Qualifications

• Bachelor of Social Work or higher; eligible for membership of the Australian Association of Social Workers.

Responsibilities

• Conduct comprehensive psychosocial assessments to identify consumer strengths, needs, risks and factors impacting mental health, wellbeing and recovery.
• Deliver evidence-based psychosocial and psychological interventions to support moderate to severe mental health challenges.
• Develop, implement and review person-centred care plans promoting recovery, resilience, independence and community participation.
• Undertake risk assessment, safety planning and ongoing monitoring to support wellbeing and manage risks.
• Provide care coordination and case management to ensure timely, integrated and appropriate supports.
• Engage with families, carers and significant others to support recovery and ongoing wellbeing.
• Assist consumers to navigate health, community, housing, legal, financial and social support systems.
