Job summary

Coast & Country Primary Care (CCPC) invites applications for a Social Worker (Mental Health) to join our multidisciplinary team delivering Psychological Therapies for Underserviced Populations across Dubbo and surrounding region. You will assess needs, provide recovery-oriented psychosocial support, and coordinate care with families and community services.

The role requires a Bachelor of Social Work, 3+ years of relevant experience, and willingness to travel Western NSW.

Qualifications

• Bachelor of Social Work or higher eligibility for AASW
• Minimum three years post-qualification experience in mental health or related human services
• Experience in mental health, trauma, counselling or psychosocial interventions is desirable
• Experience with complex social determinants and navigation barriers is desirable
• Ability to establish and maintain stakeholder relationships
• WWCC and CRC checks within last 12 months, valid Driver's license
• First Aid and CPR certificates
• Willingness to travel throughout Western NSW

Responsibilities

• Conduct comprehensive psychosocial assessments to identify needs, risks and factors affecting mental health and recovery.
• Deliver evidence-based psychosocial interventions for moderate to severe mental health challenges.
• Develop and review person-centred care plans to promote recovery and community participation.
• Undertake risk assessment, safety planning and ongoing monitoring.
• Provide care coordination and case management for timely, integrated supports.
• Engage families, carers and others, recognizing their role in recovery and wellbeing.
• Assist consumers to navigate health, housing, legal, financial and social supports to reduce barriers to access.
