Coast & Country Primary Care in Bathurst NSW is seeking a Mental Health Nurse to join a multidisciplinary team delivering Psychological Therapies for Underserviced Populations across Bathurst, Orange and surrounding regions.

You will conduct assessments, deliver brief interventions, manage risk, promote wellbeing, and coordinate care with other health professionals in a patient-centred approach.

Qualifications

• Bachelor of Nursing or equivalent qualification.
• AHPRA registration and current authority to practice as a Nurse with NMBA.
• Preferably a minimum of three years (postgraduate) clinical experience within the mental health sector.
• Demonstrated commitment to a consumer-focused recovery-oriented approach and collaboration in care planning.
• Ability to establish and maintain relationships with stakeholders.

Responsabilités

• Conduct comprehensive mental health assessments to identify consumer needs, risks, strengths and goals.
• Deliver brief interventions, psychoeducation and structured psychological strategies.
• Undertake ongoing risk assessment, monitoring and safety planning.
• Support the physical health and wellbeing of consumers through screening and referral.
• Assist consumers with medication education, monitoring and support with prescribing clinicians.
• Develop, implement and review person-centred care plans reflecting consumer needs and preferences.
• Provide care coordination and facilitate access to health, community and psychosocial support services.
