Job summary

Coast & Country Primary Care (CCPC) is seeking a Social Worker (Mental Health) to join a multidisciplinary team delivering Psychological Therapies for Underserviced Populations across Bathurst and surrounding regions. You will provide holistic, recovery-oriented social work services, address psychosocial needs, and advocate to promote positive outcomes through care coordination and community supports.

Qualifications

• Bachelor of Social Work or higher and eligibility for AASW membership.
• Minimum three years’ post-qualification experience in mental health or related human services.
• Experience in mental health, trauma, counselling, family work, AOD or psychosocial intervention is desirable.
• Experience navigating complex social determinants and service navigation barriers.
• Proven ability to establish and maintain relationships with stakeholders.
• WWCC and CRC within the last 12 months; valid driver’s license and access to a vehicle.

Responsibilities

• Conduct comprehensive psychosocial assessments to identify needs and risks.
• Deliver psychosocial and psychological interventions for moderate to severe mental health challenges.
• Develop and review person-centred care plans to promote recovery and community participation.
• Undertake risk assessment, safety planning and ongoing monitoring.
• Provide care coordination and case management for timely, integrated supports.
• Engage with families and carers to support recovery and wellbeing.
• Assist consumers to navigate health, housing, legal, financial and social supports.
