Job summary

Coast & Country Primary Care (CCPC) in NSW seeks a Mental Health Nurse to join a multidisciplinary team delivering psychological therapies for underserved populations across Bathurst, Orange and surrounding regions. The role focuses on assessment, intervention, care planning and coordination to promote recovery and wellbeing.

You must be registered with AHPRA, hold a Bachelor of Nursing, and have at least three years postgraduate mental health experience.

Qualifications

• Bachelor of Nursing or equivalent qualification and AHPRA registration.
• Minimum three years postgraduate clinical experience within the mental health sector.
• Commitment to a consumer-focused recovery-oriented approach and collaboration in care planning.

Responsibilities

• Conduct comprehensive mental health assessments to identify consumer needs, risks, strengths and goals.
• Deliver brief interventions and psychoeducation to support recovery.
• Undertake ongoing risk assessment, monitoring and safety planning.
• Support the physical health and wellbeing of consumers through screening and referrals.
• Assist with medication education and monitoring in collaboration with prescribing clinicians.
• Develop, implement and review person-centred care plans.
• Provide care coordination and facilitate access to health and psychosocial services.
