Job summary

Coast & Country Primary Care (CCPC) is seeking a Mental Health Nurse to join a multidisciplinary team delivering Psychological Therapies for Underserviced Populations Program across Dubbo and surrounding region.

You will perform assessments, deliver interventions, coordinate care and promote recovery and wellbeing for adults with moderate to severe mental health challenges, including risk management and health promotion. A full range of professional and organizational support is provided.

Qualifications

• Bachelor of Nursing or equivalent qualification.
• AHPRA registration and current authority to practice as a Nurse with Nursing and Midwifery board of Australia.
• Demonstrated commitment to a consumer-focused recovery-oriented approach and collaboration in care planning.
• Demonstrated ability to establish and maintain relationships with key stakeholders.

Responsibilities

• Conduct comprehensive mental health assessments to identify consumer needs, risks and goals and inform care planning.
• Deliver brief interventions, psychoeducation and structured psychological strategies to support recovery and improve mental health outcomes.
• Undertake ongoing risk assessment, monitoring and safety planning for consumer wellbeing and manage clinical risk.
• Support the physical health and wellbeing of consumers through screening, health promotion and referrals.
• Assist consumers with medication education, monitoring and support in collaboration with prescribing clinicians.
• Develop, implement and review person-centred care plans and provide care coordination.
