Job summary

Life Without Barriers is seeking two Lived Experience Peer Practitioners to join the Wellness Connect Commonwealth Psychosocial Support Program in the Southern Adelaide area. This permanent part-time role (0.8 FTE, 30.4 hours per week) focuses on delivering recovery-oriented psychosocial support in community and home-based settings for clients ineligible for NDIS services.

You will work with participants, families and external stakeholders to build capacity, foster community connections and help

Qualifications

• Deliver recovery-focused psychosocial support to people experiencing mental health challenges.
• Facilitate both one-to-one support and group-based recovery activities.
• Develop, implement and review person-led support plans and wellbeing goals.

Responsibilities

• Deliver recovery-focused psychosocial support to participants.
• Facilitate individual and group recovery activities.
• Develop and review person-led support plans and wellbeing goals.
• Build positive relationships with participants, families, carers and community stakeholders.
• Support participants to strengthen daily living skills and community engagement.
• Maintain accurate case notes, reports and participant records.
• Collaborate within a multidisciplinary team and external networks.
