Job summary

Ochre Health Medical Centre Sippy Downs is seeking a highly skilled Lead Nurse to coordinate patient services and provide advanced clinical leadership within a GP setting. The role focuses on improving outcomes through CDM/veteran affairs expertise and effective recall/reminder systems.

Applicants must hold current unconditional registration with AHPRA as a Registered Nurse and have prior experience in a general practice environment. Knowledge of cold chain and stock management is essential.

Responsibilities

• Coordinate patient services and act as a liaison between services, patients and the GP.
• Lead the nursing team and support the Practice Manager and Medical Coordinator.
• Provide leadership and manage performance & development of staff.
• Perform clinical duties required within a General Practice setting.
• Ensure standards and legislative requirements are met and maintained.

Scope:

Provide a link between services, patients and the general practitioner by coordinating patient services

Proven track record in improving clinical outcomes with your advanced knowledge specifically in regards toCDM/veteran affairs and Recall/ reminder systems

Assist the general practice to improve the health outcomes of its patientsby implementing, developing and providing clinical support

Ensure standards and legislative requirements are met and maintained whilst managing the clinical environment

Provide leadership and management support in consultation with the Practice Manager and Medical Coordinator

Proficient in performing the clinical duties required within a General Practice setting

Previous management experience in a general practice setting: managing the team as well as Performance & Development processes.

Current unconditional registration with AHPRA as a Registered Nurse

Prior experience working in a general practice setting

Understanding of cold chain processes

Experience with managing clinical stock and stock ordering

Understanding of the importance and functionality of recall processes in a general practice setting.

Special interest in Care Planning
