Gateway Health in Wodonga is seeking a part-time Aboriginal and Torres Strait Islander Case Manager to deliver culturally safe, strengths-based support. You will assess clients’ circumstances and collaboratively develop an Action Plan with referrals to relevant services alongside a supportive Child and Family Health team.

Qualifications

• Qualifications in welfare, community development or human services fields (Diploma minimum).
• Knowledge of Aboriginal/ Torres Strait Islander communities and relevant organisations in Wodonga/Wangaratta and related health issues.
• Demonstrated ability to work with Aboriginal people and families and provide case management.

Responsabilités

• Deliver culturally safe case management and advocacy to Aboriginal and Torres Strait Islander Peoples.
• Assess client circumstances and collaboratively develop an Action Plan with supported referrals.
• Work with a supportive Child and Family Health team to deliver services.
