Gateway Health is hiring a part-time Aboriginal Health Case Manager in Wodonga to deliver culturally safe case management and advocacy for Aboriginal and Torres Strait Islander Peoples. You will assess client circumstances and collaboratively develop an Action Plan with referrals to relevant services.

Qualifications

• Qualifications in welfare, community development or the human services fields. (Diploma qualification a minimum)
• A sound knowledge of Aboriginal/ Torres Strait Islander communities and relevant organisations within Wodonga and Wangaratta, and a demonstrated understanding of health and wellbeing issues affecting Aboriginal and Torres Strait Islander people
• Demonstrated ability to work with Aboriginal people and families and provide case management.

Responsibilities

• Deliver culturally safe case management and advocacy to Aboriginal and Torres Strait Islander Peoples.
• Assess clients’ circumstances and collaboratively develop an Action Plan including referrals to relevant services.
• Support the team of Child and Family Health professionals and engage with local Indigenous communities to improve health and wellbeing.
