Albury Wodonga Aboriginal Health Service (AWAHS) is seeking an experienced NDIS Support Coordinator to join the Social Emotional Wellbeing team. If you enjoy helping people navigate the NDIS, coordinating quality supports and empowering participants to achieve their goals, we would love to hear from you.

This Aboriginal and/or Torres Strait Islander Identified Position supports community across North East Victoria and the Southern Riverina.

Qualificações

• This role requires experience in NDIS support coordination or working with people with disability.
• Demonstrated knowledge of NDIS guidelines, legislation and community services.
• A strengths-based, culturally responsive approach is essential.
• Ability to work with a wide range of stakeholders and networks.

Responsabilidades

• Coordinate personalised supports for participants and families.
• Support access to the NDIS, including eligibility and plan implementation.
• Build strong relationships with participants, community and service providers.
• Advocate for participants and help them achieve their goals.
• Collaborate across health, disability and community services.
• Identify service gaps and help improve access pathways for the community.
