Job summary

Ability Action Australia is seeking a Behaviour Support Practitioner to support the Gunnedah region. You will oversee assessment, development and delivery of evidence-based plans for NDIS participants, driving meaningful goals within a multidisciplinary team.

Qualifications

• Degree in an allied health profession.
• Experience in report writing.
• Strong organisational skills and ability to manage a varied caseload.
• Proactive, self-driven approach with a drive to improve outcomes.
• Genuine passion for working in the disability sector.

Responsibilities

• Support NDIS participants and families to identify needs via assessment and person-centred planning.
• Own assessment, development and delivery of evidence-based support plans within a complex environment.
• Drive meaningful goals using a positive behaviour support framework and reduce restrictive practices.
• Ensure plans reflect each participant's goals, strengths and priorities.
• Build capability and confidence of support staff in participants' day-to-day lives.
• Collaborate within a multidisciplinary team to deliver holistic care.

In this role you will:

• Supporting NDIS participants, along with their families and carers, to identify their needs through assessment and person-centred planning
• Owning the assessment, development and delivery of evidence-based support plans within a complex support environment
• Driving meaningful goals and measurable outcomes, using a positive behaviour support framework and a commitment to eliminating restrictive practices
• Ensuring plans reflect each participant’s goals, strengths and what matters most to them
• Building the capability, confidence and understanding of those supporting participants in their day-to-day lives
• Working collaboratively within a cohesive multidisciplinary team environment

Qualifications
