Job summary

Housing Plus is seeking a dedicated Support Coordinator to join the housing team in Mudgee. The role focuses on case management with tenants, developing support plans, coordinating referrals and maintaining records to sustain tenancies.

We require at least 2 years in community services, strong case management knowledge, and the ability to work with vulnerable groups including Aboriginal and Torres Strait Islander communities. A current driver’s license is essential.

Qualifications

• Minimum 2 years’ experience in community services or client support and intervention
• Demonstrable knowledge of case management principles
• Knowledge of the community services sector and available services for clients in the Bathurst area
• Experience working with vulnerable people and Aboriginal and Torres Strait Islander community members
• Ability to develop and maintain productive relationships with key stakeholders including referral partners
• Ability to work effectively as part of a team and contribute to team success
• High degree of initiative and the drive to achieve set KPIs, high levels of productivity, achievement orientation
• Highly process-oriented.

Responsibilities

• Conduct regular assessments and develop tenant support plans
• Make referrals to appropriate support partners
• Build positive collaborative partnerships with relevant community support services
• Attend interagency meetings
• Maintain accurate records and case notes
• Proactively work with the Housing team to identify risks and barriers
