Job summary

Connections Case Management Pty Ltd in Canberra seeks a dedicated Support Coordinator to work closely with NDIS participants, families and providers to build capacity and coordinate supports aligned with NDIA plans.

You will manage a varied caseload, develop person-centred plans, and advocate for positive outcomes while collaborating with an experienced team to deliver high-quality, client-focused services.

Qualifications

• Experience in roles such as Support Coordinator, Case Manager or Social Worker.
• Strong understanding of NDIS and disability sector.
• Excellent communication and organisational skills; trauma-informed practice.

Responsibilities

• Conduct needs assessments with participants based on NDIS goals and plans.
• Develop a support plan with participant and family.
• Coordinate services defined in the NDIA plan with a strength-based approach.
• Help participants understand and implement their NDIS plans.
• Build participant capacity to manage supports independently.
• Develop and maintain relationships with participants, families, guardians and providers.
• Monitor supports, identify risks, and assist with problem-solving.
• Maintain accurate case notes, documentation and reporting.
• Advocate for participants to achieve positive outcomes.
• Provide person-centred or family-centred support as needed.
