As a Support Coordinator, you will work closely with NDIS participants, their families, and service providers to build capacity, coordinate supports, and help participants achieve the outcomes outlined in their NDIS plans. You will manage a varied caseload and have the autonomy to build strong relationships while being supported by an experienced and collaborative team.
Key responsibilities
- Conduct a needs assessment with the participant, based on their NDIS goals and the level of service defined in the NDIA plan
- Develop a support plan in conjunction with the participant and their family where appropriate, that clarifies the actions to be taken and outcomes to be achieved
- Coordinate services as defined in the participant's NDIA plan, using a strength-based and capacity building approach, including informal supports, mainstream services, specialist services, and NDIS funded supports
- Support participants to understand and implement their NDIS plans
- Build participant capacity to manage and navigate their supports independently
- Develop and maintain strong relationships with participants, families, guardians, and providers
- Monitor supports, identify risks, and assist with problem-solving when challenges arise
- Maintain accurate case notes, documentation, and reporting
- Advocate for participants to achieve positive outcomes
- Provide person-centred and where appropriate family-centred support based on developing skills, competencies and resources
About you
- Previous experience as a Support Coordinator, Specialist Support Coordinator, Case Manager, Social Worker, Allied Health Professional or similar role
- Qualifications in working in health, mental health, social services field or extensive experience
- Strong understanding of the NDIS and disability sector
- Experience supporting participants with complex needs
- Excellent communication and relationship-building skills
- High-level organisational and time management skills
- A commitment to person-centred and trauma-informed practice
- The ability to work both independently and as part of a supportive team
- NDIS Worker Screening Check
- Working with Vulnerable People (WWVP) registration or ability to obtain, current driver's licence, reliable vehicle, and right to work in Australia
