Job summary

Premier Supported Living Group is seeking an experienced Support Coordinator / Case Manager to join our Melbourne team. You will coordinate supports for NDIS participants, including hospital transitions, SIL, and community services, liaising with families, clinicians and service providers to achieve meaningful outcomes.

The role requires strong organisation, communication and problem-solving skills, with a participant-centred approach to planning and reviewing supports in line with NDIS Practice

Qualifications

• Experience in Support Coordination or case management within disability services.
• Strong knowledge of the NDIS and participant funding arrangements.
• Proven ability to coordinate complex supports across multiple providers.

Responsibilities

• Manage a caseload of NDIS participants and oversee support requirements.
• Coordinate transitions from hospital into community-based accommodation and supports.
• Assist participants to understand and implement their NDIS plans.
• Maintain accurate participant files and documentation.
