Job summary

Dolphin Wellness Care in Sydney, NSW is seeking a Community Services Coordinator to coordinate SIL, daily living and in-home aged care supports. This full-time role focuses on assessments, service plans and rostering to ensure safe, goal‑oriented outcomes for clients.

You will liaise with clients, families and health professionals, monitor progress and ensure compliance with NDIS and aged care standards. A permanent FT position with salary AUD 85,000–92,000 plus super.

Qualifications

• Experience in service coordination or case management is required.
• Knowledge of NDIS and/or aged care systems is essential.
• Strong communication, organisation and report-writing skills.
• Current checks and driver's license (or willingness to obtain) are required.

Responsibilities

• Conduct needs assessments for SIL, daily living, and in-home aged care clients.
• Develop, implement and review individual service plans aligned with client goals.
• Coordinate service delivery, rostering, and staff allocation.
• Liaise with clients, families, guardians, plan managers, and health professionals.
• Monitor service quality, progress toward goals, and budgets.
• Prepare service agreements, progress reports and case notes.
• Identify and respond to risks, incidents and complaints per standards.
• Build links with community organisations to boost participation.
