Qualifications

• Certificate IV Community Services or equivalent demonstrated experience in a similar role.
• Experience with Home Care Packages or Commonwealth Home Support Programme.
• Excellent computer skills with MS Office.

Responsibilities

• Provide service coordination, assessment, care planning and review for the non-clinical components of support programs.
• Coordinate the Model of Care to deliver customer-centric quality care services.
• Ensure reporting and documentation accuracy in the Client Management System.
• Manage a prescribed caseload within budgetary constraints of the service users.
• Maintain high quality customer service and effective communication with clients and families.
