Job summary

Bolton Clarke Residential Aged Care Opportunities – Brisbane QLD is seeking a Full-Time Care Coordinator to join our Home and Community team at the Caboolture Office. This fixed-term contract runs until May 2027, offering an opportunity to help older adults live independently in their own homes while coordinating care services.

You will coordinate care planning, involve funding programs, and ensure compliant documentation, with strong customer service, communication, and computer skills in MS

Qualifications

• Certificate IV Community Services or equivalent demonstrated experience in a similar role.
• Strong understanding of client needs across funding programs (e.g., Home Care Packages).
• Excellent computer skills with MS Office demonstrated.
• Commitment to high-quality customer service with clear communication.
• Strong organisational and time-management abilities.

Responsibilities

• Provide service coordination, assessment, care planning and review, for non- clinical component of support programs.
• Coordinate the planning and implementation of a Model of Care which provides customer-centric quality care services.
• Ensure reporting and documentation requirements are met, including timely administrative and communication systems.
• Provide practical assistance relating to meeting quality and audit requirements.
• Manage a prescribed caseload including assessment and care planning within budgetary constraints.
• Monitor the accuracy of electronic records in the Client Management System.
