You will bring a Certificate IV in Community Services or equivalent, strong MS Office skills, and excellent communication and organisational abilities to deliver high-quality client care.

Qualifications

• This role requires a Certificate IV in Community Services or equivalent experience.
• Strong computer skills and proficiency in MS Office.

Responsibilities

• Coordinate service delivery and care planning for non-clinical support programs.
• Plan and implement a customer-centric Model of Care.
• Ensure timely reporting and documentation in systems.
• Assist with quality, audits and compliance.
• Manage a caseload within program budgets and timelines.
• Maintain accurate client time records in the management system.
